This agreement serves to confirm that I have chosen to participate in the Kansas State University DirectLink program effective today.

By my signature below, I acknowledge that my participation in the DirectLink program requires that I:
• Consider applying for admission to Kansas State University.
• Follow the guidance of my academic advisors to ensure a successful transfer.
• Maintain good academic standing while perusing your desired degree(s) at both your current institution and K-State.

As required by the Family Educational Rights and Privacy Act of 1974 (FERPA), I authorize Kansas State University and my current institution to share my FERPA-protected University records (class schedules, grades/GPA, academic transcripts, academic standing, test scores, residency status, student ID). This authorization shall remain in force for the duration of my participation in the K-State DirectLink program, or until I submit to K-State's Office of Recruitment and Admissions a written and signed notification rescinding my permission to release the records noted, whichever should come first.
