Upon the submission of this form, the following next steps will take place:

1) The person you listed as a parent/guardian will be sent a permission form for them to acknowledge your intent to enroll in K-State courses.
2) A permission form will be sent to your counselor
3) A request for your counselor to upload your high school transcripts will be included.

Once we have received the parent/guardian permission form, the counselor permission form, and a copy of your high school transcript, we will review your application for admission to Kansas State University.
Note: Any courses requested on this form are not guaranteed. Approval is dependent on a number of criteria including admission to the university, counselor approval, course availability, completion of prerequisites courses, and academic performance on prior college work.
